The children who need our care and support are at the heart of everything we do. By looking at their individual needs and preferences, we aim to enhance their life through fun, stimulation and enjoyment.
We’re dedicated to giving every child the chance to socialise, connect and enjoy shared experiences - we work closely with you and your child to develop plans that capture your whole family's wishes, hopes and dreams, and do what we can to help you achieve them.
Even when dealing with a very serious illness, there can still be opportunities to create treasured memories.

The goals the children develop could be around:
The CHAS transition worker will liaise with other care providers and support organisations, including local authorities to do all they can to help our children achieve their goals as adults.
If you're interested in hearing more about our transition work or feel that you have a skill or service that could assist please get in touch - we'd love to chat.
