Criteria for Referral

If you are thinking of making a referral to CHAS, either for a family you know or are working with or for your own child, the following criteria will help you to establish if CHAS will be able to help.Photo by Paul Hampton

 Is the child:

  • Under 16 years of age? (This may be wavered depending on your circumstances)
  • In need of palliative care? This may be at diagnosis, mid point in the disease process, or in the end stages.
  • Experiencing a deterioration in their condition?
  • Diagnosed with an illness, where survival to adulthood is unlikely?
