Hospice Referrals
Anyone can refer a child or young person to CHAS, providing the referral has been discussed and agreed by the family.
Our services are free to children, young people and families who need them.
If a family contacts us directly with a referral request for their child we will always discuss this with the child’s lead professional and other practitioners who are proving care and advice to the family.
When we receive a referral from a family we may contact them to arrange a home visit to establish what support they are looking for. The referral is then discussed by CHAS hospice staff including doctors, Directors of Care, Deputy Directors of Care and other senior members of the team. The staff will make their decision dependent on the family’s needs and the stage of the child’s condition.
Once the decision has been made we will then contact the family detailing the level of support we are able to offer them.
If the family wish to appeal a decision once received please direct this in writing to:

All such appeals will be reported to the Clinical Governance Committee of the Board of Directors. This policy was approved by the CHAS Board of Directors in November 2006.
